Instructions for completing the application form

• Decide if you want to complete the form by hand or using a word processor
• Download the appropriate application form (see choices below)
• If you wish, get advice on what you should put on the form
• If you downloaded a .pdf form print it out and complete it by hand
• If you downloaded a .docx form complete it using a word processor and then print it out
• Sign the completed form and if you are under 18 get a parent or guardian to sign it as well
• Scan the signed form and email it to admin@cysf.org.uk
